What is wrong with this order?

Jardiance 25mg, 1 tablet by mouth daily at bedtime:

• a medication that is designed to help reduce blood sugars in those with diabetes by excreting glucose through the urine
• has indications to help with kidney function and with heart failure
• giving this medication in the evening can cause a patient to be awake during the nighttime hours and increasing the risk of a fall

Returnable Medications

Certain forms of medication cannot be returned to the pharmacy for credit since they can only be used for one person once opened. These could include inhalers, eye drops, creams, ointments, liquids, insulins or controlled medications. Consider having the physician sign an order allowing the resident to take these medications with them upon discharge from the facility, providing the order is still active and appropriate.

Crushed Medications:
“The crushing of tablets or capsules for which the manufacturer instructs to “do not crush” requires further investigation. Some exceptions to the “Do Not Crush” instruction include:

• If the prescriber orders a medication to be crushed which the manufacturer states should not be crushed, the prescriber or the pharmacist must explain, in the clinical record, why crushing the medication will not adversely affect the resident. Additionally, the pharmacist should inform the facility staff to observe for pertinent adverse effects.
• If the facility can provide literature from the medication manufacturer or from a reviewed health journal to justify why modification of the dosage form will not compromise resident care.”

Handwashing helps prevent illness. It involves five simple and effective steps (Wet, Lather, Scrub, Rinse, Dry) to reduce the spread of diarrheal and respiratory illnesses. Regular handwashing, particularly before and after certain activities, is one of the best ways to remove germs, and prevent the spread of germs to others. It’s quick, it’s simple, and it can keep us all from getting sick.
